Cash, check or credit card.
Payment is due at the time of service.

DO YOU CHARGE FOR MISSED APPOINTMENTS?

As long as you cancel within 24 hours, there is no charge. Otherwise, there is a $90 no-show fee.

DO YOU TAKE INSURANCE?

I am currently in-network with Blue Cross Blue Shield, United Healthcare and Aetna. However, if requested, I can provide a monthly billing statement to be submitted to providers for out-of-network benefits.
